Complete WebAdvisor forms

Before you are cleared to register, you will need to fill out the required Registration Readiness WebAdvisor forms:

• Emergency Notification Information 
• Measles (MMR/MMRV) Vaccination Information
• My Emergency Contacts (General Profile) 
• Linfield Educational Services Agreement (LESA)
• Disclosure of Directory Information 

How to Complete Registration Readiness Forms:

1. Login to WebAdvisor
2. Fill out all forms under General Profile section
   • Emergency Notification Information
   • Measles(MMR/MMRV) Vaccination Information (Contact your health provider for dates)
   • My Emergency Contacts (must be completed every 60 days / within 60 days of registration)
3. Fill out form under Financial Profile Information
   • Linfield Educational Services Agreement
4. Fill out form under FERPA Forms section
   • Disclosure of Directory Information
