Make a Formal Report
Please fill out the below form to make a report of sexual misconduct or relationship violence. You have options in how you make a report. If you do not feel comfortable filling out the below form, there are other ways you can make a report.
Responses submitted in the below form are received by Linfield's Title IX coordinator with Grand River Solutions.
After you submit your report, the Title IX coordinator will follow up with you regarding your report. Please identify your preferred way to be contacted. If you are concerned that someone may be monitoring your email or voicemail, you can choose to be contacted by phone only.
