Forms

The following forms are necessary for Linfield grant applicants both before and after funds are awarded.

  • Financial Conflict of Interest - With this form, all grant applicants declare whether they have a significant financial interest that impedes the objectivity of the potentially funded research or project.  
  • Payroll Check Request - Applicants must fill out this form and submit it to Human Resources in order to receive stipends through a grant. 
  • Proposal Approval - All grant applicants must fill out this form in accordance with the institution's proposal approval policy.
  • Time and Effort Reporting - All federal grants require any personnel paid through such funds to submit reports detailing the percentage of effort spent on that grant versus other duties.
